I am dedicated to working myself out of a job from the first session. I think people don't need to spend years finding out how they got toilet trained. History is important, but current problems, short term therapy and going right to the problem is a more important focus. I ask people where it hurts, and respond to their pain accordingly. After 47 years in the field, I have encountered most issues. My fees are high but if a patient is not getting a lot out of the session---right away they are probably in the wrong place.
